STEP 3:  Mail this form, your total deposit (write to Wisconsin Baha'i School Committee) and filled out sponsorship forms for children/youth to—David Martin, Registrar, 3114 Patty Ln, Middleton, WI  53562. 

 

1.  Children without a parent must also be accompanied by a sponsor attending the school. 

2.  You will be officially registered when your check is received.
